I hereby declare that the above statements are true and complete. I also declare that I will not be traveling against the advice of a physician or for the purpose of obtaining treatment. I am not on the waiting list for any medical treatment and have not received a terminal prognosis for a medical condition before this date. I understand that the policy does not cover pre-existing medical conditions either declared or undeclared. I understand, that the entire amount of the initial premium needs to be paid for the insurance policy to come into force. That I need to ensure that my said account needs to be funded with the appropriate amount to enable the bank to carry out the standing instruction. I agree to keep the bank fully indemnified and held harmless at all times, from and against all claims, expenses, losses, damages, costs (including reasonable attorney's fees), actions, suits and proceedings arising due to non-execution or delays in execution of my standing instruction either on account of non-availability of sufficient funds in my account or due to delays in mail / courier service of for any other reason beyond the control of the bank. I hereby authorise the bank to debit my above mentioned bank account/Credit Card and pass on the proceeds to the insurance company. This debit will stay authority in force till such time that I make a written representation to cancel the said instruction. The renewal premia is subject to change as per the relevant age band and service tax rates, amended by Govt. of India, Ministry of Finance. I am aware that Citibank will earn 15% of net premium as commission on this policy in their capacity as the licensed Corporate Agent of RSA. I hereby authorise M/s. Paramount Health Services Pvt. Ltd. with its cooperation partner Euro Alarm, to process claims and receive reimbursement proceeds from Royal Sundaram Alliance Insurance Company Limited.
